Services

·Comprehensive Care for acute illnesses such as colds, sore throats, joint injury/pain

·Comprehensive Care for chronic conditions/issues such as medication refills, ADD/ADHD follow up, acne treatment, treatment of asthma

·Physical exams, well woman exams, sports physicals

·Routine laboratory testing

Eligibility

·Students 11 23 years of age who possess a valid ID card

·Parents of Middle Schoolers must accompany student to appointment at Mannheim High School

Charges

·Care is FREE for all children of active duty and retired military sponsors

·Sports/Youth Services physicals are FREE regardless of pay status

·Immunizations:FREE if required for school

·Laboratory testing done in conjunction with a physical will be charged

·Children of DOD civilians are charged at the Teen Clinic Services rate.Consult the hospital cashier for details at 06221-17-2664/DSN 371-2664.

·Contact Linda Scheuermann, School Nurse, 0621-720-0524/DSN 380-9181 for an appointment at the Mannheim High School Teen Clinic.

Location

·The Teen Clinic is located at Mannheim High School, Room 130.

Dear Parent or Sponsor:

Beginning this week, the Teen Clinic at Mannheim High School will see middle school students for medical services.

The Teen Clinic is available to middle school students who possess a valid ID card.Parents must accompany students to appointments at the Teen Clinic.Students who need sports physicals may be seen unaccompanied with signed parent permission.

The Teen Clinic provides many services including:

·care for acute illness, such as colds, sore throats, minor injuries; 

·care for chronic conditions such as medication refills, ADD/ADHD follow-up, asthma;

·physical exams, sports physicals;

·routine laboratory tests;

·immunizations required for school (tetanus, hepatitis B, PPD).

Appointments are available for middle schoolers on Wednesdays from 1230 1445.
